Pool & Spa Permit checklist for Miramar

All required applications based on scope of work (Structural, Plumbing, Electrical, etc.)
Plans — 1 original set signed & sealed by PE or RA (if 11"x17" max size); if plans exceed 11"x17", then TWO (2) sets required plus 2 boundary surveys
Affidavit of Identical Documents (for digitally signed/sealed plans)
Full scalable updated Boundary Survey (1 set if plans 11x17; 2 sets if plans larger; within 7 years; original s/s)
One (1) set of all applicable specifications: pool safety devices, alarms, pool cover, heat pump, etc.
Safety Barrier Affidavit (Land Development Code Sec. 22-141(c) — barrier required around pool OR premises)
Affidavit of Awareness of Homeowner's Association (MANDATORY even if NOT in HOA)
Construction Debris Removal Affidavit (MANDATORY per City Code §18-7)
Job value including labor & materials for permit valuation
Broward Health Department approval (COMMERCIAL POOLS ONLY)
Notice of Commencement — recorded — required at first inspection when value > $5,000
